Town of Scituate Flu Vaccine Clinic for Residents Age 18 and Older

Wednesday, October 21, 2020- 10:00 AM-12:00 PM and 1:00 PM-2:00 PM

SCITUATE HARBOR COMMUNITY BUILDING 44 JERICHO ROAD SCITUATE  

You must have an appointment to attend the clinic. Regular flu vaccine is available. We have a limited number of high dose flu vaccine.

Please call the Scituate Public Health Department to schedule an appointment 781-545-8725 or email boardofhealth@scituatema.gov  

Please bring the completed attached flu vaccine release form and health screening questionnaire with you the day of your appointment.

(These forms are also available at Board of Health office. Please call 781-545-8725 before you come to the Board of Health Office) 

Due to COVID-19 anyone attending the flu vaccine clinic must wear a face mask, have an appointment and if driving to the clinic must remain in your vehicle until you have been notified by workers to enter the building.

Please complete the attached flu vaccine form and bring with you the day of the clinic along with the health screening questionnaire and all of your insurance cards. If possible, please wear a short-sleeved shirt.

Do not attend the flu clinic if you have any of the following symptoms:

FEVER or CHILLS, MUSCLE ACHES, FATIGUE, SORE THROAT, CONGESTION OR RUNNY NOSE, COUGH, SHORTNESS OF BREATH, NAUSEA OR VOMITING, DIARRHEA, LOSS OF SMELL, ALTERED OR LOSS SENSE OF TASTE 

Do not attend the clinic if you have traveled outside the country or Massachusetts 14 days prior to the flu clinic date and to a state other than Connecticut, New York, Maine, New Hampshire, Vermont, District of Columbia (DC).
